Cardiac PA \ NP

Location: Milwaukee, WI

The physician assistant or nurse practitioner works directly with the Physician to provide service to the patients in clinic and hospital setting.
This role will support the Cardiovascular Services Program at Saint Joseph.

Assist physician in the development of an integrated Heart Failure/Pulmonary Hypertension program. Will assist with Inpatient HF Service program by offering pre-discharge consultation with NP or PA.

Provide patient and family education regarding cardiovascular disease, risk factors and lifestyle modifications, procedures and protocols

Interview patient on admission, complete physical, and review with physician.

Daily rounds with SOAP on all hospitalized patients

Act as a resource for hospital staff to facilitate collaboration and communication with physician regarding patient care issues.

Discharge planning.

Dictate the hospital admission history and physical exam, consultations, as directed.

Dictate hospital discharge summaries as directed.

Follow-up on patients post discharge by telephone calls, especially those with Heart Failure/Pulmonary Hypertension diagnosis in order to prevent readmissions.

Hold Heart Failure clinics for those patients who require close follow-up to maintain compliance with care plan.

Assist billing personnnel with documentation of weekly admissions, consults , procedures and hospital rounds

Position does require weekend coverage 5-6 times per year with 2 cardiologists. This position covers all hospitals Wheaton cardiologists cover when on weekend coverage.

Skills/Requirements

Physician Assistant or Nurse Practitioner, preferably with one-two years cardiology clinic and hospital experience. PA Masters Preferred or APNP certification.
